City of Pompano <br /> eryMeacan Management FL W.1. 1O'BRIE 'S <br /> Disaster Recovery B Services <br /> Our electronic debris management system was developed to conform to US Army Corps of <br /> Engineers technical specifications and FEMA documentation requirements,and it meets or exceeds <br /> all of the detailed specifications found in the USACE electronic debris management system <br /> requirements. <br /> Paper Load Tickets <br /> if the City prefers paper load tickets,Witt O'Brien's is also proficient with the use of the standard <br /> paper load ticketing system.The paper load tickets would serve as the basis for payment for the <br /> removal contractors and would be carefully handled and managed by Witt O'Brien's.Load tickets <br /> would document and certify the loading location of the debris,its eligibility under FEMA guidelines <br /> and its type and quantity.Our Field Supervisors would manage quality control over the proper <br /> completion of the load tickets with all required information and ensure that tickets are organized, <br /> secured,sorted,recorded,compiled and distributed daily. Each ticket would contain the following <br /> information: <br /> • Prime,sub-contractors'and client's names <br /> • Truck driver's name <br /> • Truck number on the certification placard <br /> • Time and date of both the loading and disposal of the debris <br /> • Loading location (street name, nearest intersection and debris zone) <br /> • Type of debris(i.e.,vegetative,C&D,mixed,stump,hangers,leaners,white good,etc.) <br /> • Diameter of tree stump <br /> • Certified volumetric capacity of the truck in cubic yards <br /> • Estimated percentage of capacity for the load <br /> • Disposal site name or location <br /> • Monitors'names and signatures for loading and disposal <br /> • Ticket number(preprinted sequentially) <br /> • Disaster number or name <br /> • Federal Aid Roadway or other <br /> • Pass number If on Federal Aid Roadway <br /> Load Tickets would be in five carbonless copy parts: <br /> • Part 1- Disposal Site Monitor(Client Copy) <br /> • Part 2 - Driver or Contractor's on-site representative(Contractor Copy) <br /> • Part 3- Driver or Contractor's on-site representative (Contractor Copy) <br /> • Part 4-Driver or Contractor's on-site representative (Contactor Copy) <br /> • Part 5-Load Site Monitor(Witt O'Brien's Copy) <br /> Witt O'Brien's would organize tickets by date and number and at the completion of the project <br /> would provide a full set of original (Part 1) tickets as well as scanned copies of each.All voided or <br /> lost tickets would also be logged for auditing purposes. <br /> September 4,2014 Page 133 <br />
