Civil/Environmental Engineering <br /> RFQ#12-04-02 <br /> Asbestos Surveys <br /> • Receipt of task order. <br /> • Selection of staff (including licensed asbestos contractor and other credentialed person- <br /> nel). <br /> • Development of work plan, costing, and scheduling. <br /> ' • Approval of work plan, budget, and schedule by City. <br /> • Site visit including collection of samples of suspect asbestos materials. <br /> • Analysis of samples by a certified laboratory. <br /> • Interpretation of results and development of a remediation plan, if necessary, or certifica- <br /> tion the facility is asbestos free. <br /> • Preparation of report and submittal to the City for review. <br /> • Completion of the task within scope (budgeted schedule). <br /> Monitor Well Installation <br /> • Receipt of task order. <br /> • Selection of task members (including Florida registered water well contractor). <br /> • Development of work plan, costs, and schedule. <br /> • Approval of proposed plan, budget, and schedule by the City. <br /> • Schedule all team members. <br /> ' • Prepare health and safety plan. <br /> • Mark drilling area with a white outline. <br /> • Obtain all required permits. <br /> • Call for utility survey to include the area outlined 7 days before work is schedule. <br /> • Travel to site. <br /> 1 • Have safety meeting with all parties. <br /> • Examine proposed well locations to assure there are no obstructions or utilities. <br /> • If necessary, adjust well locations in consultation with project manager for the City. <br /> • Set up on drilling location. <br /> • Remove pavement, if necessary. <br /> • Hand auger or posthole dig to approximately 5 feet in depth. <br /> • Begin drilling and collect cuttings for disposal as appropriate for contamination levels. <br /> • Collect any soil samples from this location as specified in the work plan, and prepare ap- <br /> propriate geologic logs. <br /> ' • Field screen soil samples with flame ionization detector according to work plan. <br /> • Select sample depth intervals in accordance with work plan and field screening. <br /> • Collect appropriate soil samples, store in laboratory-provided glassware, and cool with <br /> ' ice. <br /> • Drill holes to final well depth according to work plan, enlarging the boring diameter, if <br /> needed. <br /> ' • Install well screen and riser according to work plan. <br /> • Complete well annulus consisting of a sand pack extending above the screen, a seal <br /> above the sand pack of hydrated bentonite or fine sand, and cement grout from the seal <br /> to the surface. <br /> • Allow time for grout to begin to set. <br /> • Oversee driller, develop the well, and record observations. <br /> ariCh—Erwin° merrlal 32 <br /> Inc. <br />
