<br />Acosta Tractors, Inc.: <br />a) Item #35 on the bid form calls for 8" DIP sanitary sewer, where is this pipe <br />reflected on the plans? <br />Response: See revised sheets in this addendum. <br />b) Item #41 on the bid form calls for 3" DIP sanitary sewer, where is this pipe <br />reflected on the plans? <br />Response: Within the lift station before connecting to 6" force main; <br />however, the bid item has been removed with a lump sum payment <br />as part of the total lift station package. <br />c) Item #23, 24 and 25 which add up to 28 structures, I was only able to <br />identify 24 structures on the plans. <br />Response: See revised bid form attached to this addendum. <br />d) Per my past experience the City requires a minimum of 5hp for the lift <br />station. <br />Response: A 5hp 3 phase pump is now specified. <br />e) In reference to question 0 & P (Addendum No.3) in the past the City <br />provided the RTU and set up communication, will this now be provided by <br />the pump manufacturer? <br />Response: This is the responsibility of Miami-Dade Water and Sewer <br />Department. <br />f) Will bid date be extended; I am still pending on several subcontractors <br />quotes. <br />Response: The bid date has been extended as provided in <br />Addendum No.4. <br />g) On Atlantic Blvd. between 282 & 288 there are two Australian Pines of <br />approx. 4' in diameter and over 100 feet tall. There is no line item for this <br />removal. <br />Response: All trees, landscaping, driveways, and other <br />improvements that are in conflict with the proposed improvement <br />shall be removed by the contractor with costs included in the <br />contract price. <br />h) The fence that is on the right-of-way 333 Atlantic Ave will that be removed <br />by the owner? <br />Response: Anything within the right-of-way shall to be removed by <br />the Contractor. The owner may salvage, at their expense, any <br />improvement, including landscaping and trees within the right. <br />Budqet Construction: <br />a) Please clarify the discrepancy in quantity from plan and new bid sheet <br />provided in Addendum NO.3? <br />Response: Please referto current Bid Tab and Plans. <br />b) Please clarify the discrepancy in unit price and estimated total for the FPL <br />undergrounding? <br />Response: The correct amount is $500,000.00 for Unit Price and <br />Estimated Price. <br /> <br />06/08 <br /> <br />00900-3 <br /> <br />05-4893 <br />
