Daily <br />reports and GIS zone maps will be provided in <br />accordance with established standard operating <br />procedures. We will provide QA services to meet the <br />requirements of the Scope of Work issued by the City. <br />Specific work locations in the geographic area will vary, <br />and are subject to change during the course of the <br />debris recovery project. We understand it is our <br />responsibility to relocate personnel as the assignment <br />may require. <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />We will implement our Debris Management standard <br />operating procedure (SOP) and develop an Accident <br />Prevention Plan (APP) and will also provide a Health <br />and Safety Coordinator for this assignment and provide <br />a minimum of 4 hours of on the job technical and health <br />and safety (H&S) training to all QA Monitors. All <br />personnel will have the personal protective equipment <br />(PPE) identified in the City's RFP and in compliance <br />with applicable federal, state, and local regulations. <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />We will provide at least one QA Roving monitor per <br />debris zone to monitor and verify eligible debris <br />removal from their zone. Roving QAs will have all <br />required PPE and other salient equipment. <br /> <br />I <br /> <br />I <br /> <br />For the debris QA monitors, we will designate a <br />Supervisory QA monitor for a maximum of every 25 QA <br />monitors who will be responsible for assisting in <br />coordination between the City and the QA monitors on <br />his team. The Supervisory QA Monitor will manage <br />reporting of time sheets for his team, and assure <br />efficient operations and compliance with the <br />requirements of the contract. He will resolve minor, <br />informal complaints and orient new staff assigned to <br />his team. Larger problems and issues will be managed <br />by our Operations Manager so as not to impede any <br />Supervisory QA Monitor's ability to perform their <br />assigned inspection duties. <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />M903301 / WHI <br /> <br />Understanding of the Debris Removal <br /> <br />Scope of Work <br /> <br />The QA Monitors for a debris removal project are <br /> <br />typically required to provide: <br /> <br />:J Quality assurance for debris removal and reduction. <br /> <br />:J Monitoring of the work performed by debris removal <br />contractors to assure compliance with City contract <br />requirements. <br /> <br />., Prompt reporting of deficiencies in the performance <br />of work by the debris removal contractors. <br /> <br />::J Debris Contractor Truck Certification of each <br />truck/trailer that is added into service by the debris <br />contractors by taking a physical measurement of <br />the truck bed and completing a truck certification <br />form. They will then issue a corresponding truck <br />placard for each vehicle certified, as well as a <br />HaulPass™ card, <br /> <br />:I Verify each truck/trailer that delivers to the disposal <br />site has a valid load ticket and placard. <br /> <br />. Make sure each truck bed is empty upon departure <br />form the disposal site. <br /> <br />::J Organize debris load tickets for proper storage, <br />tracking and quality control. All paper load tickets <br />are scanned and the digital image transmitted to the <br />database, <br /> <br />::I Monitor individual loads of debris to estimate load <br />quantity, complete debris load tickets and determine <br />the quantity of debris removed and reduced and <br />provide the load tickets daily to the City. <br /> <br />:J Manage questions or problems that could impact <br />work area safety and eligibility for reimbursement. <br />::I Perform "end of day" duties, such as verifying that <br />all debris hauling contractor vehicles have left the <br />site, and other duties that may be necessary to <br />secure the site. <br /> <br />::I Provide a daily inspection report.
