<br /> <br />City of Sunny Isles Beach - Professional Engineering Services <br />RFQ No: 10-04-01 Request for Qualifications <br /> <br />SR-5 / US-i Widening and Reconstruction. FDOT District VI. Florida <br />Engineer of Record for the Widening and Reconstruction of US-l from SW 264th Street to SW 232nd Avenue. This <br />project included the preparation of Roadway Plans, Drainage Improvements, Signalization, Lighting and Signing <br />and Pavement Markings. <br /> <br />SR-997/ Krome Avenue from SW 296th Street to US-i. FOOT District VI, Florida <br />This project is part of the improvements identified in the FOOT's Krome Avenue Action Plans approved by the <br />Miami-Dade Metropolitan Planning Organization in March, 1999. This portion of Krome Avenue is a north-south <br />two-lane roadway that extends approximately 3.78 miles from US-l to Avocado Drive, running through the <br />center of Florida City and the City of Homestead in southern Miami-Dade County. The corridor land use is <br />primarily industrial in the Florida City section (S.W. 352nd St to S.W. 328th Street/Lucy Street) and commercial in <br />the City of Homestead section (S.W. 328th Street/Lucy Street to S.W. 296th Street/Avocado Drive). <br /> <br />A listing of the proposed key personnel to be assigned to the City's various potential projects, including their <br />qualifications and experience that will be supporting Mr. Gil-Mera in this contract, including their past work <br />experience as it relates to their roles on this particular assignment is discussed below: <br /> <br />Antonio G. Acosta, P.E., (A&P) - Senior Principal -Mr. Acosta brings over 25 years of <br />experience in the Design of Highway Facilities. He graduated from F.I.U. in 1985 with a <br />Bachelor of Science in Civil Engineering and spent the first thirteen years of his career working I <br />for the Florida Department of Transportation District VI internal design office on the I <br />production of roadway plans including minor design and major reconstruction projects. During <br />the last two years of his career at FDOT, he worked as District VI Roadway Design Project <br />Manager where he was head of an Internal Design Team working on a large and small number l <br />of urban roadway projects similar to the ones that may be required by this contract. He's I <br />working today for FDOT District VI as the Roadway & Infrastructure Task Manager for the <br />Miami Intermodal Center (MIC). <br /> <br />Bob Perez, P.E.. (A&P) - Senior QA/QC Manager - Mr. Perez has over 40 years of experience in <br />the management, design and construction of diverse transportation project. He attained a <br />degree in Civil Engineering from the University of Oriente, Cuba and received his Florida <br />Professional Civil Engineering Registration in 1977. In roadway engineering his experience is <br />twofold, management and design. He recently retired from District Six of the Florida <br />Department of Transportation, where he was the Assistant District Design Engineer for the past <br />six years; in that position he managed, supervised and approved the design of all state highway <br />projects in the District, covering Miami-Dade and Monroe Counties. Bob will be in charge of the <br />Quality Assurance /Quality Control for this contract. <br /> <br />a <br /> <br /> <br />n <br />- <br /> <br /> <br />Alex Vi, P.E., (A&P) - Senior Project Engineer - A Civil Engineering Graduate and a Professional <br />Engineer Registered in the State of Florida. Mr. Vi has over eleven years of professional <br />experience in the fields of Design and Management of all types of Rural and Urban Roadway <br />Projects. Within the seven years working for FDOT District VI, four were spent as a Professional <br />Engineer Trainee, and two as a Project Engineer at FOOT District VI Internal Design Office. As a <br />Project Engineer, Mr. Vi was in charge of the preparation of all components of a complete <br />Contract Plans Set. These components included: Roadway, Signing and Pavement Marking, <br />Signalization, and Lighting. In 2003, he was appointed to the Department's CADD Roadway <br />Technical Advisory Committee (RTAC) representing District VI. RTAC is a perpetual statewide self-directed work <br />team representing all roadway CADD users of computer systems and programs. Each member provides technical <br />input to the direction of CADD and related production tools in FOOT.
