<br />o <br /> <br />acceptance or rejection by the City OM or other representative. Truck loading will comply with local Florida <br />Department of Transportation (FOOT) rules and regulations including weight limitations and the tarping/covering of <br />truckloads. <br /> <br />Detailed information regarding each vehicle is recorded on the Truck Measurement Record form (please see attached <br />sample). Each entry will be initialed by the City representative and QC field personnel signifying their participation and <br />agreement with the process and results. <br /> <br />The minimum information recorded on the Truck Measurement Record form will include: <br />I. Trailer number as assigned for the current project by Ash Britt. <br />2. License plate number of the haul vehicle. <br />3. Operating company (either AshBritt or a subcontractor). <br />4. Type of work the truck/trailer will be perfonning. <br />5. Name of the driver of the vehicle. <br />6. Measurements in feet of the height, width, and length of the truck or trailer bed. For accuracy partial feet are <br />converted to decimals. <br />7. Volume capacity in cubic yards of the truck or trailer bed. <br />8. Initial of the Field Monitor (or City rep) and AshBritt QC representative. <br /> <br />Trucks or other equipment designated for use under this contract for the purpose of transporting debris will be equipped <br />with one sign (vinyl placard), attached to the driver's side. Signs will be provided by us (please see attached sample). <br />Signs will be a minimum of 12" X 16", be approved by the City DM, and will prominently display the following <br />information: <br />· Prime contractor name. <br />· Subcontractor name. <br />· Truck number. <br />. Cubic yard capacity and tare weight. <br />. Name of inspector and inspection date. <br /> <br />o <br /> <br />The capacity is recorded in indelible red ink and the remaining information written in indelible black ink. <br /> <br />A series of digital photos of the truck or trailer are taken prominently displaying the City sign (placard) and hauling <br />capacity. The Truck Measurement Record, the digital photos and other appropriate support documentation are <br />combined in a notebook to create the Master Truck Record (examples are included in Section 6d). The original is <br />maintained in AshBritt's field office with a copy provided to the City and the City inspection tower inspectors. The <br />tower inspectors should compare the information in the Master Truck Record to the truck number and capacity as <br />posted on the truck or trailer to ensure accuracy. <br /> <br />AshBritt does not accepted hand-loading of trucks and trailers as an approved procedure as it creates opportunities for <br />load "irregularities" and hand-loading crews rarely carry appropriate commercial insurance, workers compensation, and <br />licensing necessary to meet AshBritt's subcontractor guidelines. <br /> <br />Collection Process <br />Collection crews will systematically traverse the streets within their assigned area and collect eligible debris from the <br />rights-of-way. Crews are instructed with regard to the following guidelines: <br /> <br />o <br /> <br />. <br /> <br />Operators will perform an inspection of their vehicles and equipment prior to starting work each day. Operators <br />will record the inspection on an inspection form. The form must remain in the vehicle at all times and be <br />produced upon demand by AshBritt management or any government authority. <br />Crew foremen will verify that all personnel on the crew are in PPE appropriate to their scheduled duties <br />Crews will position appropriate traffic control personnel and devices as directed. <br />Collection activities will normally begin on the street and at the point that they were concluded the previous day. <br />Crews will collect debris only from the ROW taking great care to protect as practicable as possible the existing <br />infrastructure. <br />Crews will collect all debris and will make a reasonable effort not to commingle vegetative and C&D or mixed <br />debris. White goods should be segregated and left for separate collection. <br />Crews will collect all large debris on each pass and will leave the work area with only small debris remaining. <br />Small debris is defined as debris that can only be collected by hand or with hand tools. <br />Crews are instructed to schedule collections around schools on the weekend if possible and avoid areas around <br /> <br />. <br /> <br />. <br />. <br /> <br />. <br /> <br />. <br /> <br />. <br /> <br />. <br /> <br />AshBritt.
