<br />Crew Leaders shall document daily production to monitor and ensure the most efficient operations. The 0 <br />information they are to record includes: <br /> <br />. <br /> <br />Cycle Times of Trucks <br />Loads per Hour <br />Production <br /> <br />. <br /> <br />. <br /> <br />Crew leaders are also required to make sure that safety gear is provided and that it is adequate for the <br />hazards involved and enforce proper use and wearing of protective gear. Accidents shall be recorded and <br />reported on the Supervisor's AccidenUlncident Investigation Report by the Crew Leaders. <br /> <br />Daily records submitted to the Operations Manager shall include: <br /> <br />. <br /> <br />Sub-contractor/Employee Name <br />Equipment Number <br />Type of Equipment <br />Hourly equipment documentation, downtime, lost time, and sick time <br /> <br />. <br /> <br />. <br /> <br />. <br /> <br />All accidenUincident reports shall be forwarded through the Health and Safety Manager to the Health and <br />Safety Officer (HSO). The HSO shall notify the OM, who in turn informs the City Operations Manager and <br />all procedures as set forth in the Ceres Health and Safety Program are followed. <br /> <br />Description of a Typical Workday <br /> <br />It will be the responsibility of the Sector Manager to schedule and coordinate the location of a particular <br />crew and equipment necessary for its job function to its location through direction to the Field <br />Supervisors. This will take place through schedule planning from the previous day. The Field Supervisor <br />will notify members of the crew of the start time, specific job function, and location where he/she is to <br />report. At the beginning of the day each field employee will sign in a daily time sheet, the location <br />according to zone (if the zone changes during the course of the day the employee will document the new <br />location), the phase of work he/she is performing, and the unit number and beginning hours of the piece 0 <br />of equipment that he/she is operating (if applicable). The employee responsible for loading trucks and <br />truck drivers will keep a running tally of the loads they complete from each particular zone over the course <br />of the day. It is then the responsibility of the field employee to perform an inspection of the piece of <br />equipment and inform the crew Foreman so corrective actions may be taken. The inspection will be <br />documented on a punch-list that is supplied on the employee's daily report. After the inspections and <br />documentation, the crew will proceed with the function of removing the debris from the zone they were <br />assigned. <br /> <br />Two flagmen will be placed on each end of the work perimeter to meter the flow of traffic that is allowed <br />into the work perimeter. If debris is forwarded across the roadway the flagmen will obstruct all traffic. <br />When the loading of a truck is completed the flagmen will also close traffic. During the work procedure, <br />the flagmen will be equipped with two-way radio headsets to communicate with each other the direction of <br />traffic. Additional trucks staged for loading will all be stationed to the side of the roadway to which they <br />are being loaded so they will not obstruct incoming traffic to the work perimeter. When the loading is <br />completed they will proceed out of the work area. <br /> <br />The trucks will be placed in single file to the rear of the Knuckleboom loader. As each truck in the queue <br />is loaded and departs for the dump-site, the next truck in line backs up to the loading perimeter. The <br />Knuckleboom loader will load from piles that are staged by two front-end loaders working ahead of the <br />Knuckleboom loader to limit the amount of movements of the Knuckleboom loader during the course of <br />the day. <br /> <br />The front-end loaders will stage the material from the boulevards on both sides of the street into staging <br />areas on the side of the street. If the crew is working in a high traffic area then this method will not be <br />incorporated - rather the staging will be done completely on one side then staged completely on the other <br />side. When the Knuckleboom loader encounters material difficult to handle (such as chunk wood), the <br />Front-end loader will assist in performing the loading. <br /> <br />There will be two laborers trained in the use of chain saws that will assist the Knuckleboom loader.
