<br />City of Sunny Isles Beach Project # 3122140000045; Newport Fishing Pier and
<br />Restaurant Construction, Sr. Inspector, Responsible The coordination for the pile
<br />driving and vibration monitoring Inspectors, reviewing and monitoring the construction
<br />documentation, ensure compliance with the contract documents and specifications.
<br />Scope of work for this project is the removal of the existing pier wood structure, deck
<br />and columns and construction, in the same footprint, of a new double Tee concrete deck
<br />over 24” pre stress concrete piles pier. Towards the East side of the pier a new
<br />restaurant will also be constructed. New electrical distribution system and transformer
<br />along with adjacent areas restoration will be included. Project Started: March 2012,

<br />FDOT Project # FM 42381015801; West Palm Beach (LAP) Seaboard Train
<br />Beautification Project. Reference: COWPB, Project Manager; Jeffrey E. Halverson
<br />(561) 494‐1092. Sr. Inspector, Responsible The coordination for the material testing
<br />with VT Inspectors, reviewing and monitoring the construction documentation, ensure
<br />compliance with the contract documents and specifications. Scope of work for this
<br />project is pedestrian and landscape improvements to the Seaboard Train Station site,
<br />including construction of a pedestrian plaza with landscaping elements, irrigation and
<br />lighting on the area immediately east of the existing Seaboard Train Station building,
<br />reconfiguration and landscape improvements to the surface parking lots on the north
<br />and south of the building, and some pedestrian improvements along the Tamarind
<br />Avenue right‐of‐way between Clematis Street and Fern Street. Project Started March

<br />I595 Express Corridor Improvement ($1.25 B) P3 Project. Alejandro Montenegro
<br />P.E: CES Consulting and Testing, Inc. Mr. Guerrero was working on this project in all (6)
<br />six segments A, B, C, D, E and the east and west greenways. His responsibilities included;
<br />quality assurance and collection, sampling and storage of concrete samples as required,
<br />also conducted liquid properties testing as required by FDOT. Project Started: October
<br />2010, Project Completed: Current
<br />MDX GEC Contract. Yan Chang P.E.: EAC Consulting Engineers, Inc. Senior Construction
<br />InspectorSupervised the construction/repair of concrete bridges and roadway
<br />improvement work for the Miami Dade Expressway Authority. Inspected structures to
<br />determine structural soundness and compliance with specifications, building codes, and
<br />other regulations. Responsible for contract compliance and documentation,
<br />communication with the contractor regarding work quality, field adjustments and non‐
<br />compliance work. Project Started: January 2008, Project Completed: July 2010.
<br />FDOT District 6 CEI Consultant Residency. John Hart P.E.: Carnahan, Proctor & Cross
<br />Consulting Engineers, Inc. Senior Roadway Inspector‐ District CEI Residency, projects
<br />involve managing portion of the District construction work program, administering
<br />projects from pre‐construction and plans review phases to certification of final estimate
<br />package. Provided inspection, testing, and reporting of multiple operations including
<br />bridge rehabilitation, concrete pavement slab re‐placement and grinding, asphalt
<br />paving, stripping, street signalization and lighting work. Project Started: December
<br />2004, Project Completed: January 2008
