c,1 fr <br /> °4j, City of Sunny Isles Beach <br /> Personnel Policy and Procedure <br /> Policy Name PERFORMANCE APPRAISALS - GENERAL <br /> Policy Number 303:1 <br /> Effective Date 10/1/04 <br /> Revision Date(s) 5/18/17 <br /> Prepared By, Title Human Resources Director <br /> Approved By, Title City Manager <br /> POLICY: <br /> It is the policy of the City that the job performance of each City employee should be on- <br /> going and be formally evaluated periodically by the employee's designated supervisor <br /> (rater) and reviewed by the department head, or next senior manager so designated, as <br /> the reviewer. <br /> PROCEDURE: <br /> 1) In coordination with the Human Resources Department, designated supervisors <br /> should complete performance appraisals upon the following occasions: <br /> a) At the end of the first three (3) months,and six (6), nine (9) and twelve (12) <br /> months of employment; <br /> b) In conjunction with the annual performance review; <br /> c) When the employee is transferred or promoted to a new job; <br /> d) When the employee is assigned to a new supervisor (rater); and <br /> e) When a reduction in staff is necessary. <br /> If a performance appraisal has been completed within one month of the above <br /> occasions, a new appraisal does not need to be completed. Between scheduled <br /> appraisals, supervisors should meet with employees on an informal basis and discuss <br /> any performance issues that require attention.
