seeking a Master's degree or higher will be eligible to receive reimbursement of up <br /> to Three Thousand Dollars ($3,000.00), per fiscal year, for the cost of tuition, <br /> books, materials, supplies and activity fees. <br /> 3) If less than one-half (1/2) of the educational funds budgeted for the fiscal year are <br /> not expended or encumbered at the end of the fiscal year, a participating employee <br /> will be entitled to receive an additional reimbursement amount, not to exceed <br /> Fifteen Hundred Dollars ($1,500.00). The maximum benefit amount shall be Three <br /> Thousand Dollars ($3,000.00), per fiscal year. <br /> 4) Participants may shall be required to reimburse the City should they leave, <br /> voluntarily or not, within twelve (12) months of receiving program money. <br /> Reimbursement amount will be pro-rated based on date of disbursement of funds. <br /> 5) An eligible employee will complete and submit an "Educational Reimbursement <br /> Request Form" (copy attached) to his/her Department Head. The Department <br /> Head will then submit the form to the City Manager, through the Human Resources <br /> Director, for approval. Forms can be obtained from the Human Resources <br /> Department. <br /> 6) The City Manager, in consultation with the Department Head and the Human <br /> Resources Director, will consider each request upon its merit. The Human <br /> Resources Director will then notify the employee, through the Department Head, <br /> of the City Manager's decision. <br /> 7) If approved, the employee will pay his/her tuition at the time of enrollment in the <br /> approved course(s). Upon completion of the pre-approved course(s) with a grade <br /> of"B" or better, the City will reimburse the employee accordingly. <br /> 8) Payments will be made in a timely fashion upon verification of submitted required <br /> documentation. <br /> 9) Employees receiving grant funds or scholarships are not eligible to receive <br /> reimbursement for those monies. <br /> 10) Courses must have started and be completed during the fiscal year of which <br /> reimbursement has been requested. Employees completing a <br /> introductory/probationary period can only submit for reimbursement courses that <br /> have started and are completed after they have successfully completed the <br /> introductory/probationary period. <br /> CODING: Words stricken are deletions,words underlined are additions. <br /> 506-1 Educational Reimbursement Program Page 2 of 2 <br /> 80 <br />
