  <br />City of Sunny Isles Beach | Invitation to Bid No. 17‐05‐01  32    <br />  <br /> The identity of all personnel who will be performing quality control inspections by name <br />and title, and verification that the person who actually performed the Work must not <br />perform quality control inspections. <br /> <br />Where the QCP is returned by the Project Manager for revisions or corrections, the Contractor must <br />resubmit the QCP within seven (7) days of receipt from the Project Manager(s), with requested <br />revisions or corrections. The Contractor must not implement any changes to its approved QCP <br />prior to review and acceptance by the Project Manager(s). <br /> <br />The Contractor must perform quality control inspections by qualified personnel (i.e. personnel <br />knowledgeable of all technical aspects of the Work, which would allow identification/discovery of <br />improperly performed services, and provide documentation of the inspection results to the Project <br />Manager(s) on a monthly basis). The documentation must be signed and dated by the inspector <br />at the time the inspection is completed. All completed inspection reports must be submitted to the <br />Project Manager(s). <br /> <br />2.32 WEEKLY WORK PLAN <br /> <br /> Concurrent with the submission of the Annual Execution Plan (AEP), the Contractor must submit a <br />work plan for the initial two weeks of Work. Subsequently, the Contractor must provide a Work <br />Plan every Thursday to the Project Manager(s), which will reflect the Work to be performed during <br />the next week, broken down by Site(s) and Task, and day(s) of the week the Work will be <br />performed. <br /> <br />2.33 MONTHLY REPORT <br /> <br />The Contractor must furnish a monthly report (“Report”) to the Project Manager no later than the <br />fifteenth (15th) of each month that must consist of five (5) parts, broken down as follows: <br /> <br /> Part 1: Prior month’s Basic Services activities accomplished, identified by park, right-of-way, <br />beach, beach access path, bus shelter or government facility and must include the date(s) the <br />Work was performed. <br /> <br /> Part 2: Prior month’s Supplemental Services activities accomplished, identified by park, right-of- <br />way, beach access path, bus shelter, parking lot, or government facility, the date(s) the Work was <br />performed and the cost(s) associated with the Work. <br /> <br /> Part 3: Prior month’s Re-Work activities accomplished, identified by park, right-of-way, beach <br />access path, bus shelter, parking lot, or government facility, the date(s) the Work was performed. <br /> <br /> Part 4: Prior month’s Additional Services activities accomplished, identified by park, right-of-way, <br />beach access path, bus shelter, parking lot, or government facility, the date(s) the Work was <br />performed. <br />
