GFA performed <br />construction materials testing, which consisted of conducting all laboratory testing at our USACE accredited laboratory as required by project specifications, <br />providing equipment and qualified engineering technicians to sample aggregate and soils, and providing a project manager/quality control manager to supervise and <br />coordinate all laboratory and field testing performed by GFA personnel. Mr. Kaub was responsible for supervising project staff and ensuring quality of service, as well <br />as performing a final review of all reports prior to submission. <br /> <br /> <br /> <br />g. <br />(1) TITLE AND LOCATION (City and State) <br /> <br />Hillsboro/Deerfield Beach Nourishment, Broward County, FL <br />(2) YEAR COMPLETED <br />PROFESSIONAL SERVICES <br />2015 <br /> <br />CONSTRUCTION (If <br />Applicable) <br /> <br />(3) BRIEF DESCRIPTION (Brief scope, size, cost, etc.) AND SPECIFIC ROLE  Check if project performed with current firm <br />Professional Geologist / Project Manager - The project consisted of the nourishment of a shoreline located in Broward County, Florida. Approximately 70,000 cubic <br />yards of beach sand was hauled in and placed. GFA performed construction materials testing, which consisted of conducting all laboratory testing at our USACE <br />accredited laboratory as required by project specifications, providing equipment and qualified engineering technicians to sample aggregate and soils, and providing a <br />project manager/quality control manager to supervise and coordinate all laboratory and field testing performed by GFA personnel. GFA provided turbidity sampling <br />in accordance with project specifications. GFA provided vibration monitoring services including a remote seismograph and biweekly reports submitted to the QC <br />Manager. Mr. Kaub was responsible for supervising project staff and ensuring quality of service, as well as performing a final review of all reports prior to <br />submission. <br /> <br /> <br /> <br /> <br />h. <br />(1) TITLE AND LOCATION (City and State) <br /> <br />Hillsboro Beach Renourishment Truck Haul Project, Town of Hillsboro Beach, FL <br />(2) YEAR COMPLETED <br />PROFESSIONAL SERVICES <br />2018 <br />CONSTRUCTION (If <br />Applicable) <br /> <br />(3) BRIEF DESCRIPTION (Brief scope, size, cost, etc.) AND SPECIFIC ROLE  Check if project performed with current firm <br />Professional Geologist / Project Manager – This project consisted of the truck haul placement of approximately +/-47,278 cubic yards of beach fill between FDEP <br />monuments R-6+750 to R-9 in Hillsboro Beach, Florida. The overall project is for approximately 2,198 linear feet. GFA provided pre and post-construction structural <br />survey services, developed a vibration control plan, monitored ground vibrations with remote seismograph systems, provided a vibration control specialist and <br />vibration technicians, and submitted biweekly reports to the QC Manager. GFA performed quality control testing and management which consisted of laboratory <br />testing services (sieve analysis, material finer than 200, carbonate testing, Munsell soil color, and free shell content). GFA also performed a Limited Soil and <br />Groundwater Environmental Site Assessment prior to the commencement of truck haul activity and after the project was completed to ensure that the on-site fuel <br />tank did not contaminate the area groundwater. Mr. Kaub was responsible for supervising project staff and ensuring quality of service, as well as performing a final <br />review of all reports prior to submission. <br /> <br />123
