DP&O will enter these tickets into a spreadsheet and/or database for tracking project <br /> quantities which is necessary for Contractor payments and FEMA reimbursement. <br /> DP&O has a well-established process and load tickets that comply with the City, FEMA, FHWA <br /> and other applicable federal, state and local agencies requirements for multiple "NCR" type forms <br /> allowing copies to be provided to the vehicle driver for documentation of the load and to the <br /> Contractor for their reconciliation process. The original tickets are maintained by DP&O on behalf <br /> of the City and are reconciled on a daily basis and entered into a database for data retention and <br /> future reconciliation of debris contractor invoices. <br /> The DP&O Load Tickets, (See Attachment 1 — Sample Load Ticket) include the following <br /> information: <br /> • Date &Time <br /> • Designation of"Push," first pass, second pass, and subsequent passes <br /> • The GPS location (mandatory) and the street address of closest property, if available <br /> • Type of Debris <br /> • Vehicle Number <br /> • Vehicle Capacity <br /> • Percent of Volume Full, to be completed by the Tower Monitors <br /> • Driver Name (printed) and signature <br /> • Field Monitor name (printed) and signature <br /> • Name of Subcontractor <br /> • Tower monitor name (printed) and signature <br /> 8.10.5 TDMS Site Selection, Site Management, Monitoring <br /> Selection and permitting of TDMS locations: DP&O can assist the City in locating land and the <br /> selection of the most cost-effective debris reduction method. DP&O will verify that a sufficient <br /> number of TDMS to accept and process all eligible storm debris are available. DP&O will <br /> document and monitor the maintenance of the TDMS entry and exit roads for the entire period of <br /> debris hauling, including stone for any roads requiring stabilization for ingress and egress. <br /> Site Management/ Security: DP&O will verify that the disposal of all eligible debris, reduced <br /> debris, ash residue and other products of the debris management will be in accordance with all <br /> applicable State, Federal, and Local Laws. <br /> DP&O will document Disposal in accordance with FEMA protocol utilizing the Debris <br /> Collection/Disposal Ticket. <br /> Disaster Program & Operations, Inc.
